HOW TO APPLY

 

· Contact Damascus Home for a

      initial phone screening.

 

· Complete face to face
assessment  and application.

 

· Provide results of HIV & TB

     Test (can be obtained thru

      Local Health Dept Services) and           

      a picture I.D.

 

· Upon acceptance to the

      Program provide $1500

      cashiers check or money order.

             (fees may be subject to change)
